New DMS Forms Available Online

HR & Disability Management Services have developed a new, innovative &interactive web base program to assist supervisors & managers in completing the mandatory documentation for Reasonable Accommodations.

Reasonable Accommodation is a process that assists disabled employees in the workplace.  The University provides Reasonable Accommodation to otherwise qualified employees who are disabled or become disabled and need assistance to perform the essential functions of their position.  Reasonable Accommodation is required and once a Reasonable Accommodation has been agreed upon, a written record must be completed.  Please see University of California Davis Procedure 81.1 (note e.)

Human Resources / Disability Management Services have developed a new, innovative / interactive web base program to assist supervisors / managers in completing the mandatory documentation for Reasonable Accommodations.  Department’s must complete these forms for all approved temporary and permanent accommodations.  The form is protected by Kerberos Login and Password.  Supervisors may access this form at the Human Resource web site (https://www.hr.ucdavis.edu/) and also at the Disability Management Services web site.  Click on the forms section and select DMS -Reasonable Accommodation Record of Action and DMS -Temporary Accommodation online form.
