To register:
Please complete the on-line registration form and submit via the web. Upon receipt of the form by WorkLife, a site list and map will be e-mailed to you at our earliest convenience. We also require that you download and sign the University waiver. You may return the waiver to WorkLife by e-mail, fax or campus mail.
