The Statement of Health process:
To apply for Supplemental Life and/or Dependent Life insurance through the Statement of Health process, submit form UPAY 850 to your Employee Benefits office. We will prepare a Statement of Health application for your completion and submission to Prudential for evaluation.

To name or change your beneficiary designation.
