How do I opt-out of or cancel an insurance plan?

Complete a UPAY 850 form, indicating the plan(s) you wish to opt-out of or cancel. Send it to your Benefits Office.

Legal Life Dependent Life Disability AD&D	Opt out is not an option	To terminate participation
