Supervisors can use this form for evaluations that are not part of the annual performance appraisal process. For example, this form can be used for probationary, limited employees or regular status employees at a time other than the annual review. This form can and has been also used for development discussions and other planning discussions where specific performance information might be useful in the process. This form can and has been used as an optional tool to assist in a variety of discussions between employees and supervisors for many years.
