Employee Performance Appraisal Report (EPAR)

 Merging Payroll Data into EPAR Form

 

  1. Download the Merge version of the EPAR form at the Human Resources website. Name the file EparMerge.doc and store it on your computer.

  2. Run PPS Decision Support Report 109 with an output of "Merge Spreadsheet" and store the file (EPAR.xls) in the same folder as the previous file.

  3. Open EparMerge.doc in MS-Word.

  4. If you get a message saying that Word cannot find the data source:
    • Tools
    • Mailmerge
    • Get Data
    • Open Data Source (then select EPAR.xls, Open, Ok)

  5. On the Merge Toolbar, select Merge to a New Document.

  6. The result will be a normal Word document with 5 pages for each employee. If all employees have the same supervisor, the entire file can be sent to the supervisor. Otherwise, you can use the MS-Word cut-and-paste tools to break the file into separate files for each supervisor.
